PRC on Wheels will be near Tacurong (Koronadal City) on May 2-3

Good news professionals and would be professionals of Tacurong! No more long, dizzying and costly land travel!

If you dread (with the ongoing road repairs) the long travel going to Davao City  just to get or renew your Professional Regulation Commission (PRC) license, here’s your chance to avail of these PRC services conveniently in Koronadal City this May 2-3, 2012!

Doctors, nurses, teachers etc -professionals which are clients of PRC

Yes that’s right, just forty kilometers away from Tacurong City, passing through well paved and traffic less highway with many public utility buses and vans going to and fro Koronadal daily!

In line with our mission to bring the services closer to our clients in Regions XII and ARMM , the Professional Regulation Commission (PRC)-Davao Regional Office, will be having its extension services on May 2, 2012 (9:00am-5:00pm) and May 3, 2012 (8:00am-3:00pm) at the Audio Visual Room (AVR) of St. Alexius College located at Gensan Drive, Koronadal City. PRC will be accepting renewal of professional identification cards of various professions, requests for certification and authentication, and applications for the Nurse Licensure Examination (first timers) and Licensure Examination for Teachers (repeaters and first timers).

We encourage all applicants for licensure examinations to avail of the on-line application system thru the internet by logging in to the PRC website, After encoding all the required information, applicants shall proceed to the nearest PRC Regional Office or venue of PRC Mobile Service for the submission and processing of applications, payment of fees and the issuance of the Notice of Admission for entry to the examination centers.

Please bring the following requirements

A. Application for licensure examination ( for first time applicants) :

• Birth Certificate from National Statistics Office (NSO) printed on security paper or SECPA
• Marriage Contract from NSO printed on SECPA (for female married applicants only)
• Original and authenticated photocopies of Transcript of Records bearing the following remarks:
o Date of Graduation (month/day/year)
o S.O. number (if applicable)
• Certificate of Exemption from S.O. – for applicants from accredited schools
• Separate copy of CHED S.O. with signature of approving authority. Extracts not acceptable.
• Diploma (if exact graduation date is not indicated in transcript of records)
• Latest three (3) pieces identical passport size colored pictures with complete name tag, white background and in proper attire (with sleeves)
• Php 900.00 – examination fee

Application for licensure examination ( for repeater applicants) :

• Photocopies of any of the following documents used in the previous examination taken, if available:
o Application Stub,
o Report of Ratings, or
o Official Receipt
o Exam Fee Php.900

B. Application for renewal of professional identification cards :

• Latest two (2) pieces identical passport size colored pictures with complete name tag, white background and in proper attire (with sleeves)
• Original and photocopy of PRC Identification Card ( PRC ID)
• Renewal fee ( based on the validity date)
• For representative – Letter of Authorization & valid ID
